Adelaide PHN is seeking applications from General Practices and other Primary Health Care Services in the City of Playford, City of Salisbury and surrounds to participate in the FDSV Pilot Project.

Applicants are strongly encouraged to read the attached Family, Domestic and Sexual Violence (FDSV) Pilot Project: GP or Primary Health Care Services Information Sheet to obtain details about the Project prior to applying.

Adelaide will provide advice of the outcome of your application by end of July 2025.

Please use the Online Tendering Forum for any questions. The forum will close at 4pm on Tuesday 17 June 2025.

Applicants should complete all mandatory fields and press submit when done. If not finished, the application can be saved for later but must be submitted before the deadline date and time.
