Eligibility:
This grant opportunity has been established as a one-off or ad hoc grant. The department considers that this is an appropriate type of selection process considering there is urgent need for the PHNs to be able to continue the provision of services for improving access to primary care by vulnerable populations. Grants under the PHN Multicultural Access Program are limited to existing PHNs only. You must be one of the 31 PHNs listed in the Grant Opportunity Guidelines to be eligible. This is in keeping with the intent of the program to provide a national network of regional organisations that commission and integrate services at the local level to address identified needs and provide support and system level integration for health services to improve coordination of care for patients.
