ABOUT MURRUMBIDGEE PRIMARY HEALTH NETWORK
MPHN is one of 31 local and independent PHNs established nationally to support the primary health care sector. MPHN works to:
1. support the development of a local sustainable health care workforce through providing quality improvement and professional development
2. ensure efficient and effective delivery of high quality patient centred care and improve health outcomes, particularly for those at-risk or vulnerable populations. Part of this role is to integrate local services and systems and improve coordination of care; and
3. continually monitor, assess and analyse the health needs of our region.
All PHNs have been set seven key priority areas guiding our work including: Mental health; Aboriginal and Torres Strait Islander health; Population health; Health workforce; Digital health; Aged care; Alcohol and other drugs.
ABOUT THIS REQUEST FOR PROPOSALS (RFP)
In 2023, Murrumbidgee Local Health District (MLHD) and Murrumbidgee Primary Health Network (MPHN) began implementation of the Living Well, Your Way (LWYW) Initiative for people with chronic disease.
As part of the initiative, a remote patient monitoring program was codesigned with a commissioned provider. The program is called Digitally Supported Self-Management for Chronic Disease (the Program). The purpose of the Program is to enable patients with chronic disease to monitor a range of prognostic health metrics independently at home, increasing their capacity to self-manage their chronic disease symptoms.
OPPORTUNITY ON OFFER
Murrumbidgee Primary Health Network (MPHN) in conjunction with the LWYW Initiative are seeking proposals from suitably experienced providers to evaluate the Program
The purpose of the evaluation is to identify successes and challenges encountered during implementation of the Program, determine if the solution was suitable for both clinicians and patients in the region, and whether outcomes for clinicians and patients were achieved. There will be a focus on understanding the costs and benefits of implementing the Program in the region.
This is an open tender process.
An online industry briefing will be held Wednesday 28 June 2023, 2:00pm to 3:00pm (AEST).
