Tender Description
The Patient Pathways Program (PPP) pilot has been developed in response to the complexity in navigating the health care system in Australia. The aim of the PPP is to build capacity, support access to existing services and medical research and engage the community in decisions about health and research.
This grant opportunity will extend the existing funded Patient Pathways Program (PPP) Pilot to 30 June 2022 and provide additional funding for these activities. An independent evaluation will be undertaken by the Department of Health. The grant contributes to the achievement of Outcome 1: Health Policy, Access and Support, Program 1.6 Primary Health Care Quality and Coordination.
