Tender Description
MPHN is seeking Expressions of Interest (EOI) from General Practices within the MPHN catchment to participate in the Murrumbidgee Winter Strategy 2021. This activity will support general practices to keep their most vulnerable patients well and reduce their likelihood of being hospitalised over the winter period.
The Winter Strategy 2021 primary care approach involves:
- identification of high risk patients within the practice;
- actively developing or updating care plans prior to the flu season (including sick day plans);
- ensuring follow-up for vaccination;
- empowering patients to manage their condition(s);
- the use of HealthPathways; and
- providing links to providers / groups that can provide support.
The overarching winter strategy will also target improved communication between acute and primary care for identified vulnerable patients and will be supported through a comprehensive marketing and media campaign promoting hand hygiene and flu vaccinations.
