CESPHN is seeking to fund primary health care services to people who are experiencing, or are at risk of primary and secondary homelessness within the Central and Eastern Sydney PHN (CESPHN) region.
Proposals for services that operate within the after-hours time period, or are able to demonstrate that they reduce the requirement for After Hours services and provide services to people experiencing or at risk of primary and secondary homelessness, within the Central and Eastern Sydney PHN are sought.
Central and Eastern Sydney PHN (CESPHN) are requesting proposals for the provision of services that improve the access, coordination, facilitation or delivery of primary health care services to people at risk of, or are experiencing primary and secondary homelessness. This may include the establishment of new services, the enhancement or extension of existing services.
Tenders close at 5:00 pm on Monday 8 January 2018. Late applications will not be considered.
