Overview
Central and Eastern Sydney PHN (CESPHN) is seeking Expressions of Interest from eligible small and solo (1-5 GP FTE) general practices located in Sydney's Inner West to participate in the Diabetes Care Clinic Program, supporting patients with prediabetes and type 2 diabetes.
Scope
The scope includes:
- Participation in a multidisciplinary model of care for patients with prediabetes and type 2 diabetes
- Connecting patients with diabetes-related allied health services delivered by Diabetes Australia, either onsite at the practice or virtually
- Providing these allied health services to patients at no cost
The program will run through to June 2027.
Funding
General practices selected to participate will receive $5,000 (excluding GST) in funding support.
