WentWest, operating as the Western Sydney Primary Health Network (WSPHN), is seeking proposals from appropriate providers for the provision of culturally appropriate health care services and to enhance and facilitate better care coordination for Aboriginal and/or Torres Strait Islander Populations in western Sydney.
This program supports Aboriginal and Torres Strait Islanders within western Sydney by providing care coordination, holistic support and navigation through the health care system through a team of an Indigenous Health Project Officer, Aboriginal Outreach Workers and Care Coordinators.
Patients of the program are required to identify as Aboriginal and Torres Strait Islander and are required to have one or more of the following chronic health conditions:
(a) Diabetes
(b) Cardiovascular
(c) Cancer
(d) Chronic Renal
(e) Mental Health
(f) Respiratory
