The Adelaide Primary Health Network (Adelaide PHN) is committed to improving the efficiency, effectiveness and coordination of primary health care services across the Adelaide metropolitan region. Spanning the region from Sellicks Hill to Angle Vale and between the foothills and the sea, the Adelaide PHN encompasses a community of some 1.2 million people.
Adelaide PHN is not a service provider but is responsible for commissioning services to best address local needs. Guided by community, clinical and stakeholder input, Adelaide PHN has a clear goal of improving health outcomes for the community. Established and funded by the Australian Government, Adelaide PHN is one of 31 PHNs now operating Australia-wide.
In 2015-16, under the Contract with the Australian Government, the Adelaide PHN has a responsibility to maintain continuity of after-hours services that are out of scope of the new After Hours Practice Incentive Program (PIP) administered by Medicare. In 2016-17 and onwards the Adelaide PHN is required to:
* Implement innovative and locally-tailored solutions for after-hours services, based on community need; and
* Work to address gaps in after-hours service provision.
This will be addressed via a commissioning approach noting that at this stage, current funding for PHNs After Hours is confirmed until 30 June 2017 only and PHNs must not commit to spend any part of the funding beyond 30 June 2017.
The purpose of the grants is to provide financial incentive to support GP practices and other primary care providers to implement innovative approaches to after-hours care between 1 July 2016 up to 30 June 2017.
