Homelessness Access Pilot Program Request for Proposal

⁠⁠⁠Overview

Country SA PHN (SA Rural Health Network Limited) is seeking to commission a service provider to establish and operate the Homelessness Access Pilot Program through this Request for Proposal (RFP).

An RFP approach has been used to encourage innovative, community-responsive solutions from across the sector, giving Applicants flexibility to propose creative service models tailored to the unique needs of their local communities.

About

For the purposes of this activity, homelessness includes people who are sleeping rough, unsheltered, accessing emergency accommodation, couch surfing, or living in unsafe or unstable housing. People at risk of homelessness include those experiencing housing insecurity, such as difficulty paying rent, facing eviction, or living in overcrowded or temporary accommodation (Department of Human Services, 2026).

Purpose

The project aims to reduce barriers to primary health care by delivering flexible, free clinical services directly to people experiencing, or at risk of, homelessness through an outreach-based model.

Scope

The scope includes:

• Design, implementation, and evaluation of an innovative outreach-based model that supports people experiencing, or at risk of experiencing, homelessness to access primary health care
• Delivery of flexible, free clinical services directly to people experiencing, or at risk of, homelessness through an outreach-based model
• Delivery of services in collaboration with community organisations to improve accessibility, engagement, and continuity of care

Contractor Requirements

• The service model must be underpinned by person-centred, trauma-informed, and culturally safe practice principles, ensuring care is responsive to the diverse needs, lived experiences, and cultural backgrounds of individuals accessing the service

Contract Terms and Conditions

• The successful Applicant will be expected to work towards service commencement by November 2026

• Applicants must complete an Establishment Plan outlining how key milestones and deliverables will be achieved to meet the required implementation timeline, to demonstrate readiness for service commencement
