NWMPHN seeks to commission one suitable organisation to provide a highly mobile two-person team to work with selected general practices within the Hume region. This team will play a key role in delivering multidisciplinary care to patients with complex needs.

The team will comprise of a clinical care coordinator and a social worker. The team will meet every week with clinicians and patients at four Hume practices. It will assist staff in assessing patient need, delivering care, supporting patient self-management, and coordinating access to other allied health and support services in the local area.

Maximum funding of $769,962.94 (ex. GST) across two years (FY25/26 and FY26/27) is available for this RFT.
