NT PHN is inviting requests for tender (RFT) to deliver early interventions for healthy ageing, slow decline and the ongoing management of chronic conditions in older Territorians.
NT wide submissions will be considered with priority given to applicants from Big Rivers, Barkly and Central regions.
Tender applications close Sunday 27 August 2023 at 5.00pm with successful activities commencing in October 2023.
A total funding pool of $800,000 is available for time limited activities delivered by 30 June 2024.
Activities in the scope of this tender could be:
*Codesign and implementation of targeted interventions to prevent, identify and reduce chronic disease and health issues, avoid inappropriate hospital admissions and improve health outcomes for the elderly.
*Support collaborative approaches between multidisciplinary teams and primary care providers
*Expand existing healthy ageing programs where relevant
*Educate primary health care providers on how to connect older Territorians with necessary psychosocial, health, social and welfare supports
*Educate family members or carers on how to manage an older person's health
An information session has been set for Thursday 10 August to answer questions and offer more information about the process.
