Tender Description

⁠⁠⁠The City of Cockburn (the Principal) is seeking an experienced and reliable Contractor to provide:

(a) Suitably experienced and qualified personnel (Service Personnel) to deliver home care and group social activity support services; and

(b) After-hours telephone support service for support worker relief cover and emergency call-outs.

Services shall be executed to the satisfaction of the Principal’s Representative in strict accordance with any resultant contract (Contract), contract specification (Specification) and contract schedule of rates (Price Schedule), along with any work instruction(s) issued by the Principal’s Representative.

Since 1988, Cockburn Care, on behalf of the City of Cockburn, has been helping aged residents (Client/Clients) and residents with disability (Client/Clients) to stay independent for longer. The aim is to support them live the life they want, in their own home and stay connected to the community.

The services that are available to Clients include:

(a) Domestic assistance (tidying and cleaning);
(b) Personal care (bathing and dressing);
(c) Shopping / social support (social outings and transport to appointments);
(d) Respite (support for Client’s carers and family in Client’s home or on social outing);
(e) Medication administration assistance (support Client in taking prescribed medication);
(f) Meal preparation (preparation of meals in Client’s home); and
(g) Group social activity support (assist with planned activities, Client transport/transfers, Client behaviour monitoring, personal care and meal assistance).

To ensure Clients receive quality, appropriate, flexible and coordinated support and assistance, the City of Cockburn (the Principal) requires suitably experienced and qualified personnel (Service Personnel) to provide home care and group social activity support services. After-hours telephone support service is also required for Support Worker relief cover and emergency call-outs.

The proposed Contract shall be for an initial period of three (3) years from the date of Contract award, with Principal instigated options to extend for a subsequent one (1) year period and up to an additional twelve (12) months after that, to a maximum of five (5) years.
