Tender Description
Gippsland PHN is seeking one or more providers to deliver early intervention activities and models of care for chronic disease management that supports healthy ageing and reduces pressure on local health services in the Local Government Areas of Bass Coast and Baw Baw.
Proposals may include delivering clinical services targeted at chronic disease management or facilitating community activities that support social wellbeing and mental health for senior Australians.
Online Tender Information Session: 2:00 pm on Monday 6th February 2023
A (non-mandatory) information session for any tenderers considering lodging a tender in response to this request for tender (RFT) will be held online (details shown above). The session will provide an overview of the procurement and will include an opportunity for participants to ask questions. The session will be recorded and distributed via the Tenderlink website as an addendum to the RFT, along with any responses by Gippsland PHN to questions posed by attendees.
