After Hours 10.1 Allied Health

⁠⁠⁠Primary Health Networks (PHNs) were established with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.

The South Eastern Melbourne PHN (SEMPHN) catchment encompasses an area comprising 1.5 million people and stretches from St Kilda to Sorrento and as far east as Bunyip, including the major population hubs of Monash, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.

From 1 July 2016, PHNs adopted a commissioning approach to procuring medical and health care services which is based on an assessment of the health and wellbeing needs of the community. This work determines the priority areas and guides the scope of the commissioning process.

After Hours (AH) 10.1 Allied Health:

This AH 10.1 Allied Health EOI is seeking interest from General Practices and General Practices within Community Health Centres in SEMPHN, to extend their onsite Allied Health service hours.

The objectives of the AH Program are to:

• Increase the efficiency and effectiveness of AH Primary Health Care for patients, particularly those with limited access to health services;
• Improve access to AH Primary Health Care through effective planning, coordination, and support for population-based AH Primary Health Care; and
• Improve the availability of AH GP services through working collaboratively with general practitioners.

SEMPHN seeks to increase effective and accessible Allied Health care services in the nominated period. The nominated period includes the defined after-hours period on weekdays, weekends, and public holidays. This activity will enable the community increased access to primary care health services.

Eligible providers are asked to propose the number of additional hours they would like to extend for each of their onsite Allied Health Services.

This EOI is open to accredited General Practices and General Practices in Community Health Centres in the SEMPHN Local Government Areas (LGAs) of:

• City of Greater Dandenong
• City of Frankston
• City of Casey
• Shire of Cardinia
• Shire of Mornington Peninsula
