Tender Description

⁠⁠⁠Monash Health are seeking suitable suppliers to be on a panel of preferred providers of brokered services in Podiatry to patients.

The scope of the project includes providing services to the following Monash Health departments and community-based programs:

  • Post-Acute Care (PAC)
  • Sub Post-Acute Care (SPAC)
  • Hospital in the Home (HITH)
  • Residential InReach (RESI)
  • Rehabilitation in the Home (RITH)
  • Complex Care
  • Disability Services
  • Residential Services
  • Transition Care Program (TCP)
  • In-Patient Mental Health Services and other settings/programs in situations where additional resources are required
