South Eastern NSW Primary Health Network (COORDINARE - SENSW PHN) invites Request for Proposals from eligible organisations to deliver and manage the Integrated Team Care (ITC) Program for Aboriginal and Torres Strait Islander people referred from general practices across the SE NSW region.
