CSAPHN is commissioning primary mental health care services for eligible people with severe mental illness who are being supported in primary care. This includes the provision of clinical care coordination which addresses both mental health and physical health needs.
People with severe mental illness who are supported in primary care require integrated services, particularly if they have persistent mental illness or complex needs. Integrated services need to encompass clinical mental health.
CSAPHN is seeking responses to this Request for Tender (RFT) from eligible organisations for the delivery of the Clinical Care and Coordination (CCC) program across the CSAPHN regions commencing July 2021. The RFT process aims to secure a fair and equitable distribution of services for clients with severe and complex mental health needs.
