Providers are invited to respond to a Request for Tender, for the provision of the Secondary Prevention of Chronic Pain for Healthy Ageing program across the EMPHN catchment.
EMPHN is seeking to commission a Service Provider/s to deliver an integrated solution (the service response) that activates a responsive provider network to achieve timely recovery from pain in the post-acute and sub-acute phase for individuals aged 50 years or over. The response will provide early identification of individuals at risk of transitioning to a chronic pain state, and harness suitable community-based services.
This RFT will require tenderers to lodge their responses in two parts:
(i) The first is the pre-qualification requirements of the Tender Response. All pre-qualification requirements must be addressed.
(ii) The second is the weighted evaluation criteria component of the tender response. All evaluation criteria components must be addressed.
Only Tenderers who meet all pre-qualification requirements set by EMPHN may submit the second component of the Tender Response. Please ensure you read all parts of the RFT to understand the process for lodging a response.
