GP Clinical Champion – Primary and Acute Integration
- Are you looking for challenging work that will better connect general practice to local hospitals?
- Are you interested in health system projects beyond direct patient care?
EMPHN is looking for a general practitioner in the EMPHN catchment to work on integration projects with EMPHN, Local Hospital Networks and participating general practices.
The GP Clinical Champion will have an interest or experience in the following:
- Partnership work with local hospital clinicians, administrators and other GPs
- Shared-care models of chronic disease management in primary care
- Reducing hospital demand for conditions that can be managed in primary care
- Advocating for and enhancing primary care
- Designing primary care education and training modules
The time requirement is about two hours per week on average. The time period is from March 2020 to September 2021. Initial months will require more hours per week, by mutual arrangement. Remuneration will be provided.
