Wentworth Healthcare Limited, provider of the Nepean Blue Mountains Primary Health Network, invites suitable, health providers to submit an Expression of Interest to facilitate the evidence based Agency for Clinical Innovation Community Chronic Pain Management Program model. The program consists of a 3 hour workshop once a week for 6 weeks to improve functional capacity through proven chronic pain self-management strategies. The program is aimed at people in the Nepean Blue Mountains region (which includes the local government areas of Penrith, Hawkesbury, Lithgow and Blue Mountains) who are living with chronic pain. Successful applicants will receive free training to deliver the program
