Tender Description
Capital Health Network (CHN), through the ACT Primary Health Network Mental Health Program, and ACT Health Directorate (ACT Health) are working in partnership to co-commission services for the new National Psychosocial Support (NPS) Measure.
The new NPS measure is to be designed and implemented in a flexible way focusing on designing services for people over the age of 18 with severe and complex mental health conditions that are not eligible for the NDIS. The services should address individual needs and complement clinical mental health and physical health services, with a focus on capacity building for daily living. Services will need to accommodate for the potentially episodic nature of the individual’s illness and avoid care coordination services. The NPS measure must not duplicate existing services within the ACT.
CHN is inviting applications from potential providers to:
* identify areas of need and the target cohort the proposed service model will address
* develop innovative and outcomes based service models to address psychosocial needs, focusing on capacity building, partnerships and integration and flexibility.
The Request For Proposal (RFP) process commences Friday, 26 October 2018.
The closing date and time for this RFP is 5:00pm Wednesday, 21 November 2018.
