Residents of Whittlesea, Mitchell and Murrindindi Local Government Areas (LGAs) in the north of the EMPHN catchment carry a notable burden of disease in the form of diabetes complications. Through consultation with stakeholders in this region, it is clear there is recognition of the issue and previous work undertaken to improve diabetes care, notably important work on agreed referral pathways.
Eastern Melbourne PHN (EMPHN) invites Tender Responses for the provision of Diabetes Diversion Innovations in Whittlesea, Mitchell and Murrindindi. The purpose of this Request for Tender is to build on this work and create capacity for partnership and innovation in the area of integrated multidisciplinary diabetes care models, where treatment can occur in a community setting with system support.
This work will also build toward the long-term outcome of improving patient outcomes, reducing the diabetes burden of disease in Whittlesea, Mitchell and Murrindindi, and reducing demand on acute services.
