This Request for Proposal is for the provision of e-Learning for general practice nurses across Central and Eastern Sydney PHN (CESPHN).
The aim of this project is to build primary care capacity to prevent and manage chronic disease through embedding effective evidence based and interactive e-Learning in general practice.
The e-Learning will be free, delivered flexibly and made accessible to all general practices nurses within the CESPHN region. It is envisaged that these resources will be interactive and subject to regular update and review in response to changes in clinical practice guidelines and related policy.
Central and Eastern Sydney PHN (CESPHN) are requesting proposals for the provision of e-Learning for general practice nurses.
The aim of this project is to build primary care capacity to prevent and manage chronic disease through embedding effective evidence based and interactive e-Learning in general practice.
The objectives of this project are:
* Enhance access to learning resources and updated content to nurses within CESPHN region
* Assist general practices to adopt a more integrated approach to patient management
* Provision of tools to enable nurses to implement quality improvement strategies tailored to general practice
* Better management of patients and improved quality of care through continuing professional development
* Enhance understanding and uptake of initiatives designed to improve management of patients with chronic conditions
* Increase ability to identify individuals at high risk of hospitalisation and facilitate early referral to appropriate interventions
CESPHN are seeking a service model that:
* Is evidence based and person-centred with optimal use of available resources, including workforce and infrastructure
* Will be available to nurses as and when they need to access, to support their work and professional interests
* Utilises adult learning principles and best practice guidelines to ensure effective learning
* Is consumer-focused and culturally appropriate
