Murray PHN invites submissions from primary care providers to respond to identified needs in Chronic Disease Management across the Region. This RFP is calling for proposals that will improve health outcomes and strengthen primary health service system responses for patients with:
* Chronic Obstructive Pulmonary Disease (COPD), and/or
* Cardio-vascular disease (CVD).
Murray PHN seeks collaborative responses from primary health service providers, General Practice, and registered training organisations to develop and maintain models of care to address the needs of patients with these chronic conditions.
Please note that there are three steps to applying for this tender:
1. Register your interest. Indicate intent to respond.
2. Complete the Pre-Qualification Form online to determine eligibility. This closes on 3 August at 4.00pm. If you have not submitted this form, you will not be eligible to submit your tender.
3. Submit Tender via the 'Response' tab Online Form and attach relevant documentation.
