Western Victoria Primary Health Network is seeking responses for the development of Primary Mental Health Stepped Care Model for Western Victoria . The project will be developed in partnership with Western Victoria PHN and the model is to be co-designed with consumers and other stakeholders across the region.
The project will deliver a range of resources that will support Western Victoria PHN to successfully build a stepped continuum of primary mental health care for the region. The model will make the best use of available workforce and technology and ensure existing mental health services funded by Western Victoria PHN, primary care and local models of care will be taken into consideration, and incorporated where appropriate.
The Project will include co-design activities and public consultations and be developed to meet the following outcomes:
* provide a 'blue print' for the PHN in the implementation of Primary Mental Health Stepped Care Model for Western Victoria
* describes a continuum of primary mental health services including the range of service types that are available within the local region to better match with individual and population need;
* identification of the barriers and enablers to the implementation of the Model that ensure the deployment of the stepped care model of care across the region.
The project will be conducted between 13th June until the 28th of September 2017 including co-design engagement and report writing.
