South Eastern Melbourne Primary Health Network (SEMPHN) invites General Practices to apply for a grant to expand their current capacity to improve care coordination for patients with chronic and complex conditions, within the south eastern Melbourne region.
To be eligible for a grant, your organisation must be able to demonstrate commitment to achieving the following outcomes by the end of the grant period. Your practice will be required to have:
* Effective care coordination strategies in place for people with complex and chronic conditions and;
* The capability to provide care coordination to a minimum of 30 patients with chronic and complex conditions.
