Tender Description
The Northern Territory Refugee Health Program (the Program) is funded by the Northern Territory Government (NTG) Department of Health to provide comprehensive and coordinated primary health care services for refugees in their first 12 months of settlement in Darwin. Northern Territory PHN (NT PHN) has a Service Agreement with the NTG to deliver the Program from 1 July 2016 to 30 June 2018.
NT PHN invites suitably qualified individuals, organisations, practices and consortia to apply for:
Refugee Health Program Nurse Coordinator - Provision of care coordination of primary health care services for newly settled refugees (first 12 months) and integration into mainstream Australian health care system. This includes partnerships and collaboration with health, settlement, social and community services, and delivering client health literacy and community and health workforce education for best practice refugee health.
Refugee Health Program General Practice - Provision of timely and appropriate general practice services on a bulk billing basis to all referred refugee clients, including (but not limited to):
* Initial health needs assessment
* Catch-up immunisation
* Referral to specialist services and ongoing chronic and communicable disease management
* Delivery of coordinated, culturally appropriate patient centred care.
