Tender Description
The Department of Veterans’ Affairs (DVA) invites responses from suitably qualified and experienced organisations to provide a number of important functions to support the ongoing delivery and improvement of the Coordinated Veterans’ Care Program (CVC) Program.
The CVC Program is a planned and coordinated health care model for eligible DVA Health Card (Gold) holders with one or more chronic conditions, complex care needs and who are at risk of unplanned hospitalisation.
DVA is seeking to enter into an initial three-year contract with the successful tenderer with options to extend this by a further period of two years.
