Tender Description
The purpose of Home Care Packages (HCPs) is to provide long term support to older people to remain at home, through the provision of tailored and co-ordinated packages of service.
Austin Health: is an approved provider of Home Care Packages and holds 30 Level 2 packages and 40 level 4 home care package. Eighteen (18) of these Level 4 packages are held in partnership with Melbourne Health pursuant to an agreement with the Commonwealth of Australia and Austin Health dated 16 March 2004. These packages are delivered to people living in the area Banyule, Whittlesea, Nillumbik, Darebin, Morelanf and Yarra and Hume in the Northern Metropolitan region of Victoria.
Melbourn Health: is an approved provider of Home Care Packages and holds 120 Level 2 packages. Melbourne Health administers 18 Level 4 packages. These Level 4 packages are delivered in partnership with Austin Health. Melbourne Health’s HCPs are delivered to older people residing in the Northern suburbs of Melbourne, primarily in the municipalities of Moreland, Yarra and Hume.
