East Metropolitan Health Service (EMHS) is designing and delivering a new Health Hub in Byford, Western Australia, to serve the growing community across the Shire of Serpentine Jarrahdale (SSJ). The Byford Health Hub (the Hub) is an election commitment of the Labor Government.
The Hub aims to deliver a place-based model of health and social care tailored to the needs of the SSJ. The Hub will provide the community with services located locally, who will work in an integrated way, across primary, community and specialist care. The intention is for services offered out the Hub to meet current population health and social needs, with the flexibility for the services provided and the Integrated Service Delivery Model to evolve over time as community needs change. The concept design of the Hub to date has been informed by various stakeholder inputs, including engagements with subject matter experts, multi stakeholder workshops with community members, a health and social needs assessment and review of community benefits.
An Integrated Service Delivery Model is key to achieving the vision of the Hub. The aspirational intention of the model comprises a common facility and service management, including a common reception, where possible, for a No Wrong Door approach. The Hub will have a place-based approach to links with the community, other local service providers (private and not-for-profit) and the metropolitan health services.
The new Hub will offer service providers an opportunity to:
- Utilize space within the Hub under a license and/or lease agreement; and
- Be part of an innovative integrated service delivery model that is positioned to meet the needs of the SSJ community now, and as it evolves into the future.
Potential respondents include, but are not limited to, private companies and not for profit organizations with the capability and commitment to provide the following health and social services in the purpose-built Byford Health Hub:
- Extended primary care service / Urgent Care Clinic (UCC), including basic imaging (x-ray and ultrasound) and pathology
- Enhanced primary mental health service (adult-focused)
- Community services, including:
- aged care and disability support
- allied health
- social services
- alcohol and other drug services
- financial counseling
- parenting support
- family and domestic violence support
- employment support services
- wellness groups
- other services related to health and social care
- Clinical education and training for university students, aligned to health and social services outlined above.
A briefing session will be held at Byford Secondary College , 57 Abernethy Road, Byford at 12 Oct, 2023 5:30 PM.
- Potential Respondents are encouraged to attend an in person briefing to be held at Byford Secondary College on 12 October 2023 at 5.30pm to allow time for registration, with the briefing to commence from 5.45pm. The briefing will conclude by 7pm (WST) Western Australia.
- An alternate online briefing will also be held on 19 October 2023 commencing at 5.30pm (WST) for respondents that cannot attend in person. Please refer to the RFI document for further details on how to join.
