Gold Coast Hospital and Health Service (GCHHS) is inviting relevant suppliers to respond to a Registration of interest (ROI).

GCHHS wishes to work with community Providers to explore and co-design solutions to support patients with complex care needs in the right place, which include:

1. Rapid access to respite, convalescence or carer support for clients who reside at home​
2. Accommodation and workforce with capacity and capability to meet non-acute, high care needs
3. Complex physical needs (bariatric care, wound care, tracheostomy and stoma care, enteral feeding)​
4. Progression of chronic disease​
5. End of life care ​
6. Challenging behaviours relating to cognitive impairment (adolescent, adult, aged)​
7. Additional interim and transitional care options​
8. Services that provide timely support and management of complex social needs, financial issues, legal issues, family conflict resolution/mediation, guardianship​
9. Timely and affordable access to medical, nursing and allied health assessment and management within the community.

It is anticipated that that potential Providers responding to this ROI will include:

1. Residential Aged Care Facilities and associated providers

2. NDIS Registered Providers

3. Home Care Package Providers

4. Non-governmental organisations (NGOs)

5. Private business such as primary care, community pharmacy and workforce agencies​

6. Private Hospital Providers

7. Cross sector partners such as Housing and Social Services​
