SWSLHD is seeking a service provider to administer palliative care home support services across multiple LGA’s within the south western Sydney region. The region consists of a Northern Hub (Bankstown, Liverpool,
Fairfield LGA’s) and a Southern Hub (Campbelltown, Camden, Wollondilly, Wingecarribee LGA’s). Tenderer’s have the option of submitting a proposal for both Hubs (full service coverage) or either Hub on its own.
The successful service provider will be responsible for the following;
i). Provision of a Registered Nurse (RN) to work an evening shift of 8 hours (100% of patients)
• The RN shall visit clients at home who are either in the deteriorating phase (PCOC Phase 3 and AKPS 50 or <) or the terminal phase (PCOC Phase 4 and AKPS 40 or <) of their illness
• The RN shall be able to perform clinical care including holistic symptom assessment, repositioning, personal care, management of syringe drivers, carer education and support, post-death care and Verification of Death (VOD)
• The RN shall receive clinical referrals and notes and clinical handover from the SWSLHD team providing services throughout the day
• The RN shall have access to the SWSLHD After Hours On Call Palliative Care medical specialist via phone, enabling clinical advice following escalation. Video access may be an option if approved by the palliative care clinician
ii). Provision of an Optional Care Worker (daytime) to provide personal care/some respite (minimum 60% of patients), if required
• The Care Worker shall provide personal client care (usually bed based), oral care, hair care, shaving and facial hair trimming, and carer support
• If the client dies prior to, or during the scheduled visit, the Care Worker shall be able to offer and perform post death care, including personal care and the appropriate positioning of the client, as required and within cultural practices
Coordination for the intake to the program shall be conducted via the SWSLHD Triple I (Hub). The service provider shall be responsible for the coordination of the service once a referral is received, which shall include;
• Reviewing new referrals and liaising with SWSLHD Triple I for any queries/missing referral information
• Ensuring a home visit risk and safety assessment is provided
• Planning the home visits, taking into account the client’s geographical location and clinical handover of the client’s/carer’s acuity/needs
• Ensuring the RNs record required data, complete documentation and escalate any client clinical needs or other issues to the coordinator
• Providing SWSLHD with a daily list of clients remaining on the program each evening and including those discharged from the Program through death, hospitalisation or stabilised condition
The service Provider services shall wrap around the existing services provided by the relevant nursing team within Primary and Community Health.
