Tender Description
Northern Queensland Primary Health Network (NQPHN) invites applications for the delivery of Care Finder program services for the provision of aged care specialist and intensive assistance to help people within the Care Finder target population to understand and access aged care and connect with other relevant supports in the community.
Applications are sought for the delivery of services across the entire NQPHN region. Services will be contracted by Hospital and Health Service (HHS) delivery regions and where possible grouped to improve value for money and reduce service delivery risk. Respondents will be able to nominate the HHS delivery region(s) they wish to be considered for, noting that their submission will be evaluated according to their capacity and capability to deliver the required services in the selected regions.
