Tender Description
This activity aims to meet the specialist treatment and social support needs for individuals experiencing chronic condition multi-morbidity, together with mental health and/or alcohol and other drug (AOD) related problems, where patient care can be appropriately managed through a primary care led model. Delivered by a team of registered nurses, the service will support patients who face barriers to engagement with healthcare to access specialist mental health and/or alcohol and other drug services and community social supports, through the provision of care planning and coordination, health education and navigation and support with chronic disease management.
