Tender Description
The Cancer Shared Care Model aims to provide the opportunity for a rural health service, in an area of need, to improve the experiences and health outcomes for people with cancer / cancer survivors and their carers, through the introduction of an information technology / digital solution as a shared care management platform.
Funding of up to $137,247 (excl. GST) will be provided to each of two health services (one health service will be selected from each cluster located in the Western Victorian PHN region). Funding is for a 12-month period only.
