Eastern Melbourne Primary Health Network (EMPHN) invites expressions of interest from general practices to participate in the Nurse Watch program.
In response to the COVID 19 pandemic, the nurse watch program will offer Practice Nurses training, networking and mentoring to strengthen and extend their role in managing and supporting patients with complex and chronic conditions in a telehealth environment.
The 3 month program includes: · Access to eLearning courses and virtual practice nurse networking · Individual virtual mentoring session to support with implementing the program · Program activities : - Identify patients at risk - Develop and set up clear patient pathways of care within your general practice - Engage, plan and coach your patients via telehealth - How to undertake care planning via telehealth - Maximising CDM nurse consultation MBS items Please ensure you read all parts of the EOI to understand the process for lodging a response.
