Gippsland PHN is inviting suitably qualified and experienced organisations to lodge a proposal (Proposal) to deliver a digital health solution to increase patient access to General Practitioner after-hours medical services for the postcode areas of Lakes Entrance (being, 3909) and Lake Tyers (being, 3887).
Gippsland PHN receives Commonwealth funding to work with key local stakeholders to plan, coordinate and support after-hours health services. After-hours care is defined as accessible and effective care for people whose health condition is urgent and cannot wait for treatment until regular services are next available. Gippsland PHN's After-Hours Primary Health Care Program addresses gaps in after-hours service arrangements and improves service integration in Gippsland.
The aim of the Gippsland PHN After-Hours Primary Health Care Program is to improve the health and wellbeing of people living in Gippsland in rural and remote locations.
There is an under-provision of accessible face-to-face after-hours services (and a lack of community awareness of available after-hours options) causing high level of after-hours Emergency Department presentations in Gippsland compared to the Victorian average . This has resulted in unnecessary cost for the Victorian health system, a sub-optimal experience for patients and wider community concerns about the provision of after-hours services.
There is currently limited after-hours primary healthcare services in Lakes Entrance and Lake Tyres.
Gippsland PHN and its project partners are leading a pilot project (being, the subject of this RFP process) to increase access to general practitioner after-hours primary healthcare services for the postcode areas of Lakes Entrance and Lake Tyers.
