This Expression of Interest is seeking innovative solutions for providing after hours primary health care to residents of SWS with service provision to all or parts of the seven local government areas.
Initiatives under this service type should reduce the need for, or increase access to, innovative, local after-hours models of service that provide flexible options to people across the lifespan.
Various service models will be considered these include but not limited to: collaborative models between Pharmacy and General Practice, Pharmacy and nurse-led models, walk in GP-led multidisciplinary clinics, General practice cooperative models, telehealth consultations, home visiting, blended or mixed model of services, other innovative clinical models that meet criteria.
