Building capacity to improve person centered care in general practice Tranche 2

⁠⁠⁠Eastern Melbourne PHN (EMPHN) would like to invite general practices to participate in Building capacity to improve person centered care in general practice.

The focus of this program is to enhance current capacity and support capability building within the Primary Care Nursing workforce, which will take management of chronic disease to the next level.

A suite of professional development workshops, webinar and supported mentoring sessions have been developed to build the skills of primary care nurses, to align with the Australian Government’s overall plan for how care will be delivered under the proposed HCH model.

EMPHN Facilitator will provide 5 practice support visits throughout the duration of the program and will be available as required to support the practice in patient identification, data collection and program implementation.

What’s in it for the practice?
* Support practice nurses to work with patients to build skills, confidence and trust
* Engage patients to be active in their care
* Promote patient-practice partnerships
* Better use of care coordination and chronic disease management item numbers

Participating general practices will receive 5 incentive payments to a total value of $10,000 upon attendance at workshops, webinars, teleconferences and data submission.

Please note, there is a limit of one nurse per clinic.
