WSPHN is seeking innovative proposals that address health care factors and barriers to health care services for people facing homelessness in the WSPHN catchment.
We have $100,000 to commission out to a provider(s) whose proposal(s) meet the following criteria:
Proposals must directly address the health care factors and barriers faced by those who are homeless or at risk of facing homelessness in the western Sydney catchment.
Examples of such services could be, outreach health service proposals, provision of transport to and from specialist appointments or provision of funding packages to assist with health-related issues etc.
Submissions close at 4:00 PM AEDT on Tuesday 30th April 2019 and submissions must be fully received by this time.
