Monash Health are seeking suitable suppliers to be on a panel of preferred providers of brokered services in Occupational Therapy and Physiotherapy to patients.
The scope of the project includes providing services to the following Monash Health departments and community-based programs:
* Post-Acute Care (PAC)
* Sub Post-Acute Care (SPAC)
* Community Support Options (CSO)
* Hospital in the Home (HITH)
* Residential InReach (RESI)
* Rehabilitation in the Home (RITH)
* Disability Services
* Complex Care
* Residential Services
* Transition Care Program (TCP)
* In-Patient Services (including Acute, Sub Acute, Emergency and Mental Health), in situations where additional resources are required
* Other community-based programs (including those funded under Home and Community Care and the Commonwealth Home Support Program)
