General Practice is the cornerstone of an individual's health care team. The EMPHN catchment has 394 practices of which 74.5% employ one or more nurses.
Priumary Care Nurses within the catchment have a diverse scope of practice and are well positioned to address the emerging healthcare challenges.
Building on trusted patient relationships, they have the capacity to integrate and coordinate relevant members of the broader health care team and fill the need as health coaches, educators, chronic care managers and care coordinators.
The focus of this program is to enhance current capacity, and support capability building within the Primary Care Nursing workforce, which will take management of chronic disease to the next level.
The initiative aims to build the skills of primary care nurses to align with the Australian Government's vision for how care will be delivered under the proposed Health Care Homes model.
EMPHN is seeking expressions of interest from general practices for practice nurses to undertake training through a suite of professional development workshops and supported mentoring sessions.
