The Darling Downs and West Moreton Primary Health Network (DDWMPHN) Health Needs Assessment (HNA), identified Chronic Disease as a service priority, both in the effective management of recognised chronic disease and its associated risk factors and identified that the DDWM region is higher than the state average for physical inactivity and obesity.
Self-Management Support is a recognized intervention for the management of chronic disease however the structure of our health systems provides limited opportunity for this to occur. This activity is aimed at increasing the availability and opportunity for clients with chronic disease to actively engage in the self-management of their chronic disease.
The delivery of this activity will be determined by the mode of intervention and the targeted behavior and it will be the responsibility of the service provider to clearly outline the details of the program. Some self-management programs already exist in these communities and this funding does not seek to duplicate these programs. This care will incur no out-of-pocket expenses for the client and will be provided within regular business hours. Some populations are disproportionally affected by Chronic Disease and it is a mandatory requirement for services to implement strategies which specifically target vulnerable groups to ensure equitable access to health care.
These will be pilot programs with funding up to 30 June 2018 with the possibility of extension based on provider performance, reporting evaluation and service efficacy.
