Tender Description
This program will implement care coordinator positions which target complex clients with chronic disease who require additional support to manage their chronic disease and access timely and appropriate healthcare.
These will be pilot programs with funding up to 30 June 2018 with the possibility of extension based on provider performance, reporting evaluation and service efficacy.
