Tender Description
This document is a tender brief and has been prepared to provide information to assist prospective tenderers in the preparation and submission of proposals for Ballarat Health Services Community Programs Personal and Domestic Care and Community Support Services. The scope of this RFT is for the Linkages Program, Community Aged Care Packages (CACPS) and the Extended Aged Care in the Home Packages (EACH).
Ballarat Health Services (BHS) operate a number of programs that purchase a range of community based services to support the client post their hospital admission. The community based services are delivered on a short-term arrangement or may continue on an ongoing basis to support the client to continue living in their own home as they age.
The services that are purchased include light housework, communication services, home based respite, nutrition and hydration services, personal care, medication dispensing and community access (transport).
