Rural Primary Health Service (RPHS) Sunshine Coast - Gympie LGA

⁠⁠⁠Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) is a commissioning organisation, inviting eligible organisations to submit proposals to develop and implement evidence-based multi-disciplinary allied health services, for children (3.5 - 5 years) identified as being developmentally and behaviourally vulnerable, in two or more AEDC domains.

The total funding is $120,000 (excluding GST).

The PHN is one of 31 Commonwealth funded PHNs nationwide. It is a not for profit organisation which coordinates primary and preventative health care for a region of 161,000km2, with a population of 823,985 people; stretching from Glasshouse Mountains in the south, to north of Yeppoon and west of Emerald.

The PHN is funded to increase access to evidenced-based allied health care services for children who are developmentally and behaviourally vulnerable in two or more AEDC domains, and to increase efficiency and effectiveness of allied health services particularly for those children living in the outlying areas of the Gympie LGA.

Eligibility: Individuals, companies or other legal entities who can form legally binding contracts; are over the age of 18 years; and hold an ABN. For further information refer to the PHN's Conditions of Participation, downloaded with this Notice.

Eligable Bidders must be a current provider of services for developmentally vulnerable children in the Gympie LGA.

Bids: Bids will only be accepted through the TenderLink portal.

Who should complete the bid: Organisations are encouraged to partner with other organisations when submitting a bid. In this instance the 'lead' organisation should submit the bid, and reflect partnership arrangements.

Proposed Term: The proposed term of the contract arising from this Proposal is from 01/11/2017 to 30/06/2018.

Deadline for submissions: at 17:00 (5:00 PM) QLD TIME on Thursday 5 October 2017 all submissions must be fully received by this time.
