Commissioning Mental Health Integrated Complex Care (MHICC) - Frankston

⁠⁠⁠Mental Health Integrated Complex Care (MHICC) service

This Request for Tender (RFT) is an invitation by SEMPHN to suitably qualified suppliers to submit proposals for delivering Mental Health Integrated Complex Care (MHICC) services within the SEMPHN catchment.

SEMPHN is seeking to engage one or more suppliers to deliver a MHICC service in each LGA within our catchment. Suppliers may apply to provide a MHICC service in more than one LGA.

MHICC services are designed to support consumers with a severe mental illness who have complex needs and will benefit from improved coordination of their care over long periods.

MHICC services consist of a package of four core elements, and are designed to be highly flexible in their structure and delivery, enabling them to be matched to each individual's needs.

The four elements of a MHICC service package are:
* clinical nursing services
* family support and liaison
* care coordination and liaison (clinical and non-clinical services)
* improving access to psychiatrist and psychological care

Successful applicants will be funded from 1 September 2017 and will be offered a service agreement to 30 June 2019 (subject to confirmation of funding from the Commonwealth Department of Health).
