Tender Description
This service aims to facilitate improved coordination of care for young people up to 25 years of age who have complex needs, significant AOD issues and who may need reintegration support (e.g. transitioning back into the community).
The NQPHN is committed to procuring this service in four distinct regions in the NQPHN footprint: Shire of Cook; Shires of Mareeba and Tablelands Region; Burdekin and Charters Towers Region; and Mackay, Isaac and Whitsunday Regions.
NQPHN is seeking to establish regional care coordination and case management hubs for youth, up to 25 years of age, with complex needs and AOD issues.
These regional services will assist youth with AOD issues which are placing them at significant risk. For example, young people at risk of or currently engaged with youth justice system; out of home care; residential rehabilitation centres; or correctional facilities.
This service will assertively engage the young person with a range of indicated service providers to facilitate an effective approach to supporting their recovery and social and emotional wellbeing.
