ACT PHN is seeking to procure the services of a health related consultancy to inform the development of a comprehensive patient-centred and whole of system approach to CHF care in the ACT.
To support this system innovation the following services are required:
i. Co-design and develop an evidence informed, patient-centred, general practice focused and whole-system integrated model of CHF care
ii. As a subset of (i) review palliative care approaches as part of the management of CHF within the ACT, with a view to ensuring that palliative care for patients with end-stage heart failure is an integral part of any model
iii. Develop actionable recommendations on the implementation and piloting of the agreed model
iv. Co-design and develop an ACT wide CHF specific outcomes framework and associated minimum data set.
All co-design will be undertaken in collaboration with the CHF Clinical Leadership Forum (See Attachment A) and the CHF Project Steering Group (See Attachment B) and this requirement should be built into any proposed approach.
